Description

A synthetic dimethylxanthine derivative; structurally related to theophylline and caffeine; useful in the symptomatic treatment of complications of peripheral vascular diseases.

Dosage And Indications
For the treatment of intermittent claudication associated with peripheral vascular disease. Oral dosage Adults

400 mg PO 3 times daily with meals. If the drug is not tolerated at this dosage (i.e., central nervous system and/or gastrointestinal-related side effects occur), reduce the dose to 400 mg PO twice daily. If therapy is not tolerated at this lower dose, the manufacturer recommends discontinuation of the drug. Even though some patients may have symptomatic relief after 2 to 4 weeks, treatment with pentoxifylline is recommended for at least 8 weeks to determine efficacy. The manufacturer states that efficacy of pentoxifylline has been established in well controlled studies up to 6-month's duration.

Geriatric

See adult dosage. In general, dose selection should be cautious, usually starting at the low end of the dosing range. In those elderly patients with a CrCl of less than 50 mL/min, an adjustment in the dosage interval (i.e., less frequent dosing) is recommended.

For the treatment of Behcet's syndrome†. Oral dosage Adults

In case reports, pentoxifylline 300 mg PO twice daily for 2 weeks, then 300 mg PO once daily lead to improvements in symptoms associated with Behcet's syndrome.

For the prophylaxis of acute claudication due to sickle cell disease†. Oral dosage Adults

Initially, 400 mg PO 3 times daily. Limited data from small numbers of patients have shown successful treatment outcomes with doses ranging from 1.2 to 2.4 g/day PO given in divided doses for up to 2 years. Other data indicate that pentoxifylline is of no benefit. Data on the safety of pentoxifylline at doses greater than 1,200 mg/day are limited.

For the treatment of neuropathic pain secondary to diabetic neuropathy†. Oral dosage Adults

A dosage of 400 mg PO 3 times daily has been studied.

For patients with Kawasaki disease† to reduce the incidence of coronary artery lesions. Oral dosage Children

In a trial of 22 children (mean age 2 years) with Kawasaki syndrome, treatment with pentoxifylline 20 mg/kg/day PO in 3 divided doses in combination with aspirin and IVIG resulted in a lower incidence of coronary artery lesions.

Dosing Considerations
Hepatic Impairment

Specific guidelines for dosage adjustments in hepatic impairment are not available; it appears that no dosage adjustments are needed.

Renal Impairment

Pentoxifylline and its metabolites are eliminated almost exclusively by the kidney. Dosage adjustment recommendations are based on two small, pharmacokinetic studies.
CrCl >= 50 ml/min: No dosage adjustment needed.
CrCl 10—50 ml/min: Reduce dosage interval to twice daily.
CrCl < 10 ml/min: Reduce dosage interval to once daily. If toxicity develops at this dosage, consider reducing dose to 200 mg PO once daily.

Drug Interactions

Abrocitinib: (Contraindicated) Concurrent use with pentoxifylline is contraindicated during the first 3 months of abrocitinib therapy due to an increased risk of bleeding with thrombocytopenia.
Acarbose: (Moderate) Pentoxiphylline has been used concurrently with antidiabetic agents without observed problems, but it may enhance the hypoglycemic action of antidiabetic agents. Patients should be monitored for changes in glycemic control while receiving pentoxifylline in combination with antidiabetic agents.
Acebutolol: (Moderate) Pentoxifylline has been used concurrently with antihypertensive drugs (beta blockers, diuretics) without observed problems. Small decreases in blood pressure have been observed in some patients treated with pentoxifylline; periodic systemic blood pressure monitoring is recommended for patients receiving concomitant antihypertensives. If indicated, dosage of the antihypertensive agents should be reduced.
Acetaminophen; Aspirin, ASA; Caffeine: (Moderate) The concomitant administration of platelet inhibitor with pentoxifylline in the treatment of intermittent claudication has not been evaluated and should be approached with caution, due to the potential for synergistic effects.

Chlorthalidone; Clonidine: (Moderate) Pentoxifylline has been used concurrently with antihypertensive drugs (beta blockers, diuretics) without observed problems. Small decreases in blood pressure have been observed in some patients treated with pentoxifylline; periodic systemic blood pressure monitoring is recommended for patients receiving concomitant antihypertensives. If indicated, dosage of the antihypertensive agents should be reduced.
Cimetidine: (Moderate) Cimetidine, a known hepatic enzyme inhibitor, can increase pentoxifylline serum concentrations. Patients should be monitored for increased pentoxifylline adverse reactions if cimetidine is added.
Ciprofloxacin: (Moderate) Ciprofloxacin may increase pentoxifylline serum concentrations. It has been proposed that ciprofloxacin reduces hepatic CYP1A2 metabolism of pentoxifylline. Until further data are available, monitoring for increased pentoxifylline side effects (e.g., headache) is prudent when adding ciprofloxacin to pentoxifylline.

Warfarin: (Moderate) Pentoxifylline interacts with warfarin by increasing the inhibition of platelet aggregation when the drugs are administered together. Prolonged prothrombin times have been reported when pentoxifylline and oral anticoagulants have been used concomitantly. Therefore, careful monitoring of prothrombin time, signs of bleeding, and hematocrit/hemoglobin should be done periodically.

Mechanism Of Action

Mechanism of Action: The actions of pentoxifylline include increased erythrocyte flexibility and decreased blood viscosity. The mechanism of action for increasing erythrocyte flexibility is unknown, but the drug's actions appear to be related to inhibition of erythrocyte phosphodiesterase, which causes an increase in erythrocyte cAMP activity. This increase allows the erythrocyte membrane to maintain its integrity and become more resistant to deformity. Pentoxifylline's effect on blood viscosity is attributed to its reduction in plasma fibrinogen concentrations and an increase in fibrinolytic activity, as well as to its effects on erythrocytes. Improvement in blood viscosity results in increased blood flow to the microcirculation and enhanced tissue oxygenation. Unlike theophylline, pentoxifylline does not possess any bronchodilatory actions. Although pentoxifylline does not possess any direct anti-sickling properties, its actions on erythrocyte flexibility make it potentially beneficial in sickle cell disease.

Pharmacokinetics

Pentoxifylline is administered orally.  The distribution of pentoxifylline has not been fully characterized, but it is known that the drug and its metabolites are distributed into breast milk (see Contraindications/Precautions).
 
Metabolism of pentoxifylline occurs in both the erythrocytes and the liver. All metabolites contribute to the hematological effects of pentoxifylline. The 5-hydroxyhexyl metabolite is equivalent to pentoxifylline in its pharmacologic effects. Once absorbed, pentoxifylline is metabolized rapidly by the erythrocytes to its 5-hydroxyhexyl metabolite and by the liver to its 3-carboxypropyl metabolite. The plasma half-lives of pentoxifylline and its metabolites are 0.4—0.8 hours and 1.6—1.8 hours, respectively. The elimination half-lives of pentoxifylline and its 5-hydroxyhexyl metabolite increase with increasing doses of the drug, but the elimination half-life of the 3-carboxypropyl metabolite does not. Excretion of pentoxifylline and its metabolites is primarily renal. The manufacturer states that the drug does not accumulate in plasma with multiple oral doses in patients with normal renal function.

Oral Route

Absorption of pentoxifylline from the gastrointestinal tract following oral administration is rapid and almost complete. There is significant first-pass effect after absorption. Peak plasma levels are attained within 2—4 hours. The rate but not the extent of absorption is affected by coadministration with food.

Pregnancy And Lactation
Pregnancy

Pentoxifylline is classified as FDA pregnancy risk category C. No controlled studies of pentoxifylline use have been conducted in women who are pregnant. Therefore, caution should be exercised when the drug is used during pregnancy.

Pentoxifylline and its metabolites are excreted into human milk. According to the manufacturer, pentoxifylline has been shown to be tumorigenic in rats; therefore, a decision should be made to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother. A single dose of extended-release pentoxifylline was given to 5 women who had been breast-feeding for at least 6 weeks. Data from this study indicate an exclusively breastfed infant would receive about 2% of the maternal-adjusted dosage. Consider the benefits of breast-feeding, the risk of potential infant drug exposure, and the risk of an untreated or inadequately treated condition. If a breast-feeding infant experiences an adverse effect related to a maternally ingested drug, healthcare providers are encouraged to report the adverse effect to the FDA.
