Observe and exercise appropriate precautions for handling, preparation, administration, and disposal of hazardous drugs.
Wear impervious gloves when handling lomustine capsules or bottles containing lomustine capsules. Cutting, crushing, or otherwise manipulating tablets/capsules will increase exposure.
If skin contact occurs, wash affected area immediately and thoroughly.

Oral Administration Oral Solid Formulations

Take lomustine as a single oral dose; do not repeat a dose for at least 6 weeks.
Swallow whole; do not open or crush capsules. Avoid exposure to broken capsules.
Only the appropriate number of capsules required for a single dose should be dispensed at one time; never dispense the entire lomustine bottle.
A combination of 2 or more different strength/color lomustine capsules may be necessary to make up the total single dose.

Boxed Warning
Bleeding, bone marrow suppression, chemotherapy, infection, leukopenia, thrombocytopenia

Delayed, dose-related, cumulative, and sometimes fatal bone marrow suppression/myelosuppression including severe leukopenia and thrombocytopenia has been reported with lomustine therapy. Additionally, death from infection or bleeding related to severe myelosuppression may occur. Monitor complete blood counts during therapy and for at least 6 weeks after each dose. Do not administer lomustine more frequently than every 6 weeks. Withhold subsequent doses for more than 6 weeks if needed until the leukocyte count recovers to 4,000 cells/mm3 or greater and the platelet count recovers to 100,000 cells/mm3 or greater; adjust lomustine doses based on nadir blood counts from prior dose. Myelosuppression typically occurs 4 to 6 weeks after drug administration and persists for 1 to 2 weeks. Patients with compromised bone marrow function and/or patients receiving concomitant myelosuppressive chemotherapy should start at a reduced lomustine dose.

Potential for overdose or poisoning

Lomustine has a potential for overdose or poisoning; fatal toxicity has been reported with lomustine overdose. Do not prescribe, dispense, or administer more than one dose at a time. Physicians and pharmacists should inform patients that only one lomustine dose should be taken no sooner than every 6 weeks.

Dosage And Indications
For the treatment of Hodgkin lymphoma in patients who have progressive disease following initial chemotherapy, in combination with other chemotherapeutic agents. Oral dosage Adults

130 mg/m2 (round to the nearest 10-mg capsule) orally as a single dose in combination with other chemotherapy agents; do not administer lomustine more frequently than once every 6 weeks. A lower starting dose may be required in patients with compromised bone marrow function (i.e., 100 mg/m2) or if lomustine is administered with other myelosuppressive drugs. Withholding the next dose for more than 6 weeks and a dose adjustment may be necessary in patients who develop severe myelosuppression. Subsequent lomustine doses are based on nadir blood counts from the prior dose.

Infants, Children, and Adolescents

100 mg/m2 or 110 mg/m2 orally once every 6 weeks (median of 3 cycles) in combination with vincristine and procarbazine. Treatment was started 3 to 4 weeks after completion of radiotherapy.

For the treatment of non-Hodgkin's lymphoma (NHL)†, in combination with other chemotherapy agents. Oral dosage Adults

Dosage is not established. 50 mg/m2 PO on day 1 repeated every 6 weeks with cyclophosphamide, doxorubicin, vincristine, and prednisone resulted in an overall response rate (ORR) of 79% and a median overall survival of 18.9 months in 39 patients with advanced, histologically unfavorable NHL. Ten patients in this study who had received a cumulative doxorubicin dose of 450 mg/m2 and achieved a complete or partial response also received maintenance therapy for up to 2 years with lomustine 80 mg/m2 PO on day 1 repeated every 6 weeks and cyclophosphamide on day 1 repeated every 3 weeks. In another small study, lomustine 80 mg/m2 PO on day 1 and vinblastine 6 mg/m2 IV on days 1 and 8 repeated every 4 weeks produced an ORR of 60% in 20 patients with relapsed or refractory, intermediate- or high-grade NHL. All responses occurred in patients with high-grade NHL.

For the treatment of metastatic malignant melanoma† in combination with dacarbazine-containing chemotherapy. Oral dosage Adults

80 mg PO on day 1 every 4 weeks as a component of BOLD (bleomycin-vincristine-lomustine-dacarbazine) plus interferon alfa or 1.5 mg/kg PO on day 2 every 6 weeks as a component of DCV (dacarbazine-lomustine-vincristine) have been studied. Lomustine 80 mg PO on day 1 in combination with dacarbazine (200 mg/m2/day IV on days 1 to 5), vincristine (1 mg/m2 to a maximum of 2 mg/dose IV on days 1 and 4), and bleomycin (15 mg IV on days 2 and 5) (BOLD regimen) plus interferon alfa (natural or recombinant) (3 million units subcutaneously daily starting on day 8 for 6 weeks, then 6 million units subcutaneously 3 times/week) resulted in nonsignificantly higher overall response rates (ORR) compared with dacarbazine plus interferon alfa as first-line treatment in 106 patients with metastatic melanoma in a randomized, phase II trial. Patient who received BOLD plus interferon alfa experienced significantly more hematologic toxicity, neurotoxicity, and alopecia compared with dacarbazine plus interferon alfa. Lomustine 1.5 mg/kg PO on day 2 repeated every 6 weeks in combination with dacarbazine (2.7 mg/kg IV days 1 to 5) and vincristine (0.027 mg/kg IV on days 1 and 5) (DCV regimen) resulted in an ORR of 16% in patients with metastatic melanoma in a 4-arm randomized trial (n = 243). In this study, the ORR and overall survival times were not significantly improved in patients who received combination therapy with DCV or dacarbazine alone. However, 18% of patients who had previously received dacarbazine experienced a partial response following DCV therapy.

For the treatment of acute myelogenous leukemia (AML)†. As induction therapy and post induction therapy in elderly patients with previously untreated AML without unfavorable cytogenetics, in combination with idarubicin and cytarabine†. Oral dosage Adults 60 years and older

200 mg/m2 orally on day 1 in combination with idarubicin 8 mg/m2 IV daily on days 1, 2, 3, 4, and 5 and cytarabine 100 mg/m2 IV daily as a continuous IV infusion on days 1 to 7 as induction therapy was evaluated in a randomized, phase 3 trial (the LAM-SA 2007 FILO Trial). Patients who achieved a complete remission (CR) or CR with incomplete recovery after one induction course received consolidation therapy with idarubicin 8 mg/m2 IV daily on days 1, 2, and 3; cytarabine 50 mg/m2 subcutaneously every 12 hours on days 1, 2, 3, 4, and 5; and lomustine 80 mg orally on day 1 followed by 6 reinduction courses of idarubicin 8 mg/m2 IV on day 1; cytarabine 50 mg/m2 subcutaneously every 12 hours on days 1, 2, 3, 4, and 5; and lomustine 40 mg orally on day 1. In this study, patients also received 6 months of maintenance therapy with alternating courses of 6-mercaptopurine and methotrexate.

Drug Interactions

Abciximab: (Moderate) An additive risk of bleeding may occur when platelet inhibitors are used with agents that cause clinically significant thrombocytopenia including antineoplastic agents, such as lomustine.
Acetaminophen; Ibuprofen: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. In addition, large doses of salicylates (>= 3-4 g/day) can cause hypoprothrombinemia, an additional risk factor for bleeding.
Albuterol; Budesonide: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Alpha interferons: (Moderate) Additive myelosuppressive effects may be seen when alpha interferons are given concurrently with other myelosuppressive agents, such as antineoplastic agents or immunosuppressives.

Bacillus Calmette-Guerin Vaccine, BCG: (Contraindicated) Live virus vaccines should generally not be administered to an immunosuppressed patient. Live virus vaccines may induce the illness they are intended to prevent and are generally contraindicated for use during immunosuppressive treatment. The immune response of the immunocompromised patient to vaccines may be decreased, even despite alternate vaccination schedules or more frequent booster doses. If immunization is necessary, choose an alternative to live vaccination, or, consider a delay or change in the immunization schedule. Practitioners should refer to the most recent CDC guidelines regarding vaccination of patients who are receiving drugs that adversely affect the immune system.
Beclomethasone: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Betamethasone: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.

Chlorpheniramine; Ibuprofen; Pseudoephedrine: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. Cholera Vaccine: (Moderate) Patients receiving immunosuppressant medications may have a diminished response to the live cholera vaccine. When feasible, administer indicated vaccines prior to initiating immunosuppressant medications. Counsel patients receiving immunosuppressant medications about the possibility of a diminished vaccine response and to continue to follow precautions to avoid exposure to cholera bacteria after receiving the vaccine.
Ciclesonide: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Cilostazol: (Moderate) An additive risk of bleeding may occur when platelet inhibitors are used with agents that cause clinically significant thrombocytopenia including antineoplastic agents, such as lomustine.
Cimetidine: (Major) Avoid coadministration of cimetidine and lomustine. Concomitant use of cimetidine and lomustine causes an increase in bone marrow toxicity. The mechanism of this effect is not clear, and may not be related to cimetidine's inhibition of the hepatic CYP450 enzyme system.
Clopidogrel: (Moderate) An additive risk of bleeding may occur when platelet inhibitors are used with agents that cause clinically significant thrombocytopenia including antineoplastic agents, such as lomustine.
Clozapine: (Major) It is unclear if concurrent use of other drugs known to cause neutropenia (e.g., antineoplastic agents) increases the risk or severity of clozapine-induced neutropenia. Because there is no strong rationale for avoiding clozapine in patients treated with these drugs, consider increased absolute neutrophil count (ANC) monitoring and consult the treating oncologist.
Corticosteroids: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Cortisone: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Dabigatran: (Moderate) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants.
Dalteparin: (Moderate) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants.
Deflazacort: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Desirudin: (Moderate) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants.
Dexamethasone: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.
Diclofenac: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. Diclofenac; Misoprostol: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. Diflunisal: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. In addition, large doses of salicylates (>= 3-4 g/day) can cause hypoprothrombinemia, an additional risk factor for bleeding.
Digoxin: (Moderate) Some antineoplastic agents have been reported to decrease the absorption of digoxin tablets due to their adverse effects on the GI mucosa; the effect on digoxin liquid is not known. The reduction in digoxin tablet absorption has resulted in plasma concentrations that are 50% of pretreatment levels and has been clinically significant in some patients. It is prudent to closely monitor patients for loss of clinical efficacy of digoxin while receiving antineoplastic therapy.
Diphenhydramine; Ibuprofen: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. Eptifibatide: (Moderate) An additive risk of bleeding may occur when platelet inhibitors are used with agents that cause clinically significant thrombocytopenia including antineoplastic agents, such as lomustine.
Etodolac: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. In addition, large doses of salicylates (>= 3-4 g/day) can cause hypoprothrombinemia, an additional risk factor for bleeding.
Febuxostat: (Major) Coadministration of febuxostat and cytotoxic antineoplastic agents has not been studied. After antineoplastic therapy, tumor cell breakdown may greatly increase the rate of purine metabolism to uric acid. Febuxostat inhibits uric acid formation, but does not affect xanthine and hypoxanthine formation. An increased renal load of these two uric acid precursors can occur and result in xanthine nephropathy and calculi.
Fenoprofen: (Major) Due to the bone marrow suppressive and thrombocytopenic effects of lomustine, an additive risk of bleeding may be seen in patients receiving concomitant anticoagulants, NSAIDs, platelet inhibitors, including aspirin, ASA, strontium-89 chloride, and thrombolytic agents. In addition, large doses of salicylates (>= 3-4 g/day) can cause hypoprothrombinemia, an additional risk factor for bleeding.
Filgrastim, G-CSF: (Major) Filgrastim induces the proliferation of neutrophil-progenitor cells, and, because antineoplastic agents exert their toxic effects against rapidly growing cells, filgrastim is contraindicated for use during the 24 hours before or after cytotoxic chemotherapy.
Fludrocortisone: (Minor) Because systemically administered corticosteroids exhibit immunosuppressive effects when given in high doses and/or for extended periods, additive effects may be seen with other immunosuppressives or antineoplastic agents.

Mechanism Of Action

Mechanism of Action: Nitrosourea antineoplastic agents are alkylating agents. This class of drugs work by inhibiting DNA replication, RNA transcription, and nucleic acid function. The drug is believed to inhibit DNA replication, RNA transcription, and nucleic acid function. Lomustine also may modify cellular proteins. Specifically, lomustine possesses a chloroethyl group that alkylate nucleic acids and cell proteins and form DNA-DNA or DNA-protein crosslinks, which lead to the global effects of impaired nucleic acid functioning. As compared to carmustine, lomustine is less likely to form isocyanates leading to diminished carbamoylation effects; although lomustine metabolites exhibit increased alkylating activity. Cells resistant to other alkylating agents are probably resistant to lomustine.

Pharmacokinetics

Lomustine, CCNU is administered orally. The drug distributes throughout the body tissues. Because of the high lipid solubility and the relative lack of ionization at physiologic pH, lomustine crosses the blood-brain barrier quite effectively; however, only lomustine's metabolites can be detected in the CSF. CSF concentrations are >= 50% of plasma concentrations. The half-life of lomustine metabolites ranges from 16 hours to 2 days. Following the administration of radio-labeled lomustine at oral doses of 30—100 mg/m2, about half of the radioactivity was excreted in the urine in the form of metabolites within 24 hours. Most of the metabolites are eliminated slowly in the urine.

Oral Route

The drug is rapidly absorbed across the GI tract following oral administration, or across the skin following topical administration. The exact bioavailability is unknown, but nearly complete oral absorption occurs in 30 minutes. Orally-administered lomustine is rapidly and totally converted to metabolites by first-pass metabolism in the liver. Essentially all of an oral dose is metabolized within 1 hour; some of these metabolites may be active.

Pregnancy And Lactation
Pregnancy

Fetal harm may occur if lomustine is administered to a woman during pregnancy, based on animal data and its mechanism of action. Females of reproductive potential should be advised to avoid pregnancy. If lomustine is used during pregnancy, the patient should be informed of the potential hazard to the fetus. Embryo-fetal toxicity and teratogenicity have been observed when lomustine was administered daily in pregnant rats and rabbits at doses approximately 2- to 4-times the total human dose of 130 mg/m2 over 6 weeks (0.18- to 0.27-times the single human dose of 130 mg/m2) based on body surface area. Malformations (i.e., omphalocele, ectepia cordis, scoliosis, syndactyly, hydrocephalus, microphthalmia, anophthalmia, anomalies of aortic arch, dextrocardia, malpositioning of the ovaries and testes, sternoschisis, and shortened/misshapen bone of the fore or hind limbs) and decreased fetal body weight were observed following the administration of daily intraperitoneal lomustine (at dose levels of 0, 2, 4, 6, and 8 mg/kg) in pregnant rats during organogenesis.

It is not known whether lomustine or its metabolites are secreted in human milk; additionally, its effects on the breast fed infant or milk production are unknown. According to the manufacturer, breast-feeding should be discontinued during therapy and for 2 weeks after the last dose because of the potential for serious adverse reactions in nursing infants from lomustine.
